Abstract
Introduction: The objectives of our study are to determine:1.Characteristics of these patients;2. Factors associated with survival and all causes mortality in VTE and concurrent cancer or hidden.
Material and methods: Prospective study in acute symptomatic VTE patients identified by the RIETE registry (Computerized Register of thromboembolic disease).A comparative study between-groups (previous cancer, occult cancer, concomitant cancer and patients without cancer). In patients with occult cancer and concomitant cancer we conducted logistic regression and Cox regression to identify variables associated with mortality and survival.
Results: We obtained a final sample of 52.289 patients, of whom 1.845 had concomitant cancer, and 664 hidden cancers.
In patients with concomitant cancer, there was a 9.5% recurrence and 11.5% of bleeding, died 46%, and the variables associated with worse survival were: weight (RR:0.99;CI95%:0.98-0.99), hemoglobin(RR:0.92,CI95%CI 0.87 to 0.96),leukocytes (RR:1.02;95%CI:1.01-1.03) and previous surgery (RR:0.42;95%CI:0.25-0.70).
In patients with occult malignancy, there was a 17% recurrence and 18% of bleeding, 28% died, and the variables associated with worse survival were: age (RR:1.02;95%CI:1.01-1.04), hemoglobin (RR:0.85;95%CI: 0.79-0.92), leukocytes (RR:1.07;95%CI:1.03-1.11) and placement of inferior vena cava filter (RR:2.0;95%CI:1.24-3.23).
Conclusions: This study reveals clear differences between the two groups (concomitant cancer vs occult cancer). Patients with concomitant cancer have a worse prognosis than patients with occult malignancy, although the latter have a higher number of recurrences and bleeding.
