Summary:
a) Tuberculous empyema may appear similar to empyema due to other microorganisms, with visceral and parietal pleural thickening and enhancement, but is often circumferential involving the mediastinal surface (arrows). b) A review of the lungs may provide clues to the aetiology by the presence of other signs of pulmonary TB: tree-in-bud opacification, miliary nodules and cavitation point towards the diagnosis.
