Summary: 
a) Optimal measurement of pulmonary artery wedge pressure must be performed at the last complete pressure wave at end-expiration before the onset of inspiration. Automated software usually averages the entire data set, therefore manual determination of the pulmonary capillary wedge pressure is mandatory. b) Patients with diastolic heart failure may have normal wedge pressure if taking maximal diuretics. After a fluid challenge, the abnormal rise of wedge pressure can be noted. The horizontal line in both panels is the correct wedge pressure after manual adjustment.
