Summary: 
Pathophysiology of pulmonary venous hypertension and reactive pulmonary vasculopathy. In the setting of increased left atrial pressure, the absence of valves in the pulmonary veins translates into pulmonary venous hypertension, which, in turn causes a passive increase in the pulmonary arterial pressure (Ppa) while the transpulmonary gradient remains normal. If the left atrial and pulmonary venous pressures fall, Ppa will decrease as well. Some patients with pulmonary venous hypertension develop a reactive pulmonary vasculopathy. The result is an increase in the transpulmonary gradient and pulmonary vascular resistance (PVR) with worsening of the pulmonary arterial hypertension (PAH). Right atrial hypertension occurs when the right ventricle (RV) fails. HTN: hypertension.
